Identify and analyze one of the leading causes of hospital readmissions over the past 5 years. In your opinion, outline how each of the three major stakeholders can implement a process or procedure to reduce their contribution to the readmission cycle.

One of the leading causes for hospital readmission is Sepsis which is further progressing to Severe Sepsis, Septic Shock and death of the patient.
Where as Stroke, VTE and AMI are the others that fall in to the second category.
Three stakeholders that can reduce readmission rates of hospitals are
- Modification of health care plans, providing proper care which makes the patient to be stabilized in the health condition and can optimize or balance his or her own condition.
- Hospital systems can be changed , some of the commercial hospitals provide half pledged care which makes the patient to readmit, or even though full pledged treatment was given followup policies were not maintained that makes people to readmit.
- Provider based policies and rules can be implemented, as physicians now a days in a lure of earning money practicing some strategies which are making people to readmit.
