Answer: The relationship among abstracting and generation and procedure of code includes the proper documentation and sequencing of details of diseases that has to be coded. Abstracting involves codes because clinical documentation vary and to provide better and complete details abstracting is necessary. The billing and medical codes are generated on the ICD guidelines based on the diseases, diagnoses and the treatment provided.

Primary care includes the care which is generally given on daily basis i.e. day to daycare, For example, care for monitoring blood pressure, glucose level, depression etc. Secondary care includes in which patients from primary health care are being referred to other physician or health care providers for better treatment. For example, Referring the patient to an advanced hospital. Tertiary care consist of the treatment provided such as neurosurgery, cancer management, plastic surgery, cardiac surgery, treatment for severe burns etc.

A proprietary hospital is a kind of hospital which is being operated for a profit. It is mainly a profit-making business and formed by physicians or any organization whereas voluntary hospitals are non- profitable, it is mainly aimed for charitable work.
