  • Mr. Rodriguez is a 39-yr-old man who come in to clinic today complaining "blurry vision". He is monolingual Spanish speaking and has lived in Richmond, Ca. for about 8 years. He works as an undocumented laborer and has no health insurance and cannot buy any insurance using the Affordable Care Act. He is married with 4 school age children. His vital signs are: 98.6 149/96 88 14 Ht 5' 8" Wt 190 lbs. He drinks a couple six packs of beer on the weekends. He does not smoke. When you check his random blood sugar (RBS) the result is 202.

Cultural sel assessment

Cultural self assessment is an appraisal or evaluation on one's own culture, his beliefs, customs and health behaviours that affect health. Culture can be defined as the arts, customs, lifestyle, back grounds and habits that characterize a particular society or nation.

In the case study ( above given question) , it is clearly given that the patient is a Spanish person, and he knows and use only one language . He is having a history of bad health habits like smoking and alcoholism. He is having a big family with his wife and four children. Even though he is staying in US for the last 8 years, he is still an undocumented labourer and not registered in any of the Medical plans like Medicaid or Medicare. Vital signs monitoring have shown that he is having a normal temperature, pulse and respiration. But his BP recorded as high ( 149/ 96) and the RBS ( random blood sugar level )also high( 202mg/dl ), indicating the hypertension and Diabetes Mellitus respectively. His body mass index ,  BMI is 37.1 and he is coming under obese category.

How we can help Mr . Rodriguez from assessment:

As he is a monolingual person, nurse can use an interpreter while doing the further assessment and care to understand the patient well and to deliver health care effectively. From the cultural self assessment, nurse understood about the need for educating the person on:

* the importance of healthy lifestyle and the avoidance of smoking and alcohol habits.

* proper registration of the employment and enrolling in Medicaid or Medicare

* need for controlling his body weight

* educating regarding the adverse effect of not controlling sugar level and body weight.

Information missing in self assessment:

Culturally informed care means the nursing (or medical care) that respects and acknowledges the patient's and family member's cultural beliefs, attitudes  and practices.

Informations have to be collected ,for a culturally informed care , further are:

* Cultural and special religious beliefs or practices related to care of the ill

* Nutritional pattern and family history of obesity

* family history of substance abuse and it's relation with their cultural practice.

* Family medical history and his past medical or surgical history

* Obtain family's socio economic status and literacy

* Patient's cultural and religious faith, level of acculturation etc.

3 tips, while working with Medical interpreter :

A nurse has to use some tips while working with Medical interpreter, otherwise the communication may not be effective: The tips are;

* ask one question at a time, the question should not contains multi parts and complex things

* Use small or short sentences or statement

* Avoid medical terminologies or jargon

* Don't comment on patient, he may overheard you as he knows some language

* nurse can use interpreter as a resource, in case of any query related to cultural beliefs or practices.
