On admission his Blood sugar is 532, Potassium is 7.2 and ABG results include pH 7.08, Bicarb 12, and CO2 28. His VS are HR 116, BP 107/64, RR 36 Deep and rapid, and Temp 101.5. Answer the following questions:

What interventions do expect to be ordered for Mr. Sweet? Why?

What type of acid-base disturbance does he have?

What do you think is Mr. Sweet's Medical Diagnosis?

Intervention:

1. Adequate hydration

2. Regular insulin with normal sale given through intravenous.

3. Continous blood sugar monitoring.

Why?

1. In this case scenario patient is having random blood sugar 532, patient may have ketone bodies in blood and patient will have dehydration due to polyuria and hyper thermia. So patient need adequate hydration.

2. Blood sugar around 532 is considered very high, so patient may need to receive regular insulin through intravenous route for speedy recovery.

Acid base balance:

Metabolic acidosis

Rational:

PH: 7.08 indicates acidosis

Bicorb: 12 indicates metabolism acidosis

Poco2: 28: supports metabolism acidosis
