what is the pathophysiology, clinical manifestations, nursing care, and collaborative management of a patient with acute/chronic respiratory failure with hypercapnia.
Answer :
PATHOPHYSIOLOGY OF ACUTE/CHRONIC RESPIRATORY FAILURE WITH HYPERCAPNIA :
1. PACO2 -greater than 50 mmHG.
not compensation for metabolic alkalosis.
If PAO2-PaO2 ( paratial pressure of oxygen) level is in normal it may leads to alveolar hypoventilation, which may cause central hypoventilation and neuromuscular problems.
If PAO2- PaO2 level is increased it may leads toV/Q (ventilation perfusion)abnormality which may cause hypermetabolism.
2. Decreased minute ventilation (mv) relative to demand.
Increased daed space ventilation.
etiology :
Neuromuscular transmission failure ( type 2)
myasthenia gravis.
airway failure type 2 - obstuction. and dysfunction.
Clinical manifestations :
Rapid breathing.
confusion.
shortness of breath.
daily headache.
corbondoxide in atrial blood below normal limits.
Increased cerebral flow and csf pressure.
papiloedema.
warm extremities collapsing pulse.
acidosis( respiratory and metabolic).
Nursing care and collaborative management:
medical supportive therapy;
Treating the underlying cause.
maintaining adequate cardiac output.
maintaining adequate haemoglobin concentration.
Respiratory therapy ;
mobilization of secretions, continued
hydration and humidification.
chest physical therapy.
airway suctioning.
positive pressure ventilation.
