ANSWER.

* Calculation of volume(ml) of drug required per dose.

        Formula

             Volume(ml) = Desired dose(D)/Dose in hand(H) * Quantity(Q)

Here

      Ordered dose = 0.3 mg/kg.

      Weight of the patient = 27 lb

Convert lb to kg for calculation

          1 lb = 0.454 kg

      Weight of the patient in kg = 27 * 0.454

                                                 = 12.23 kg.

       Desired dose(D) = 0.3 * 12.23

                                   = 3.66 mg.

      Dose in hand(H) = 2 mg.

              Quantity(Q) = 5 ml

As per above formula

          Volume(ml) of drug required = D/H * Q

                                                        = 3.66/2 * 5

                                                        = 1.8 * 5

                                                        = 9 ml per dose.
