A sentinel event is also called a never event . This is so called because in healthcare it should never happen, e.g the patients death or serious complication or disability due to faulty procedure or medical errors and not due to patients illness .It was first told by Ken Feizer ,MD, CEO of National Quality Forum.

Adverse events such as wrong surgical procedures like surgery in the left leg instead of right leg, as the tumor was in right leg ,wrong tooth extraction, wrong breast surgery etc which should never occur..

Example It is extremely useful for dental professionals to record the unwanted never event or sentinel perioperative event such as wrong tooth extraction or other serious never incidents. This is very important as highlighted by NHS for patient care improvement or patient care safety.In the below cited article the chronological changes made in safety procedures following untoward events in a hospital described. Ultimately a set of safety procedures (huddles)adopted in opd care where invasive oral surgical procedure were adopted under local anaesthesia.The human training with these huddles (all clinical staffs) ultimately ends in no further never events in that setting.
