ANSWER :

VENOUS ULCER : Venous ulcers are defined as the wounds which occur due to improper functioning of venous system, it usually occurs in the leg region, called as Leg Ulcers. The ulcer converts into chronic wounds if not treated promptly.

1. Locate :

ASSESSMENT FINDINGS :

*Identify surface area of ulcer by measuring length and width of ulcer wound

*Diagnose venous ulcers using clinical examination measurement which is taken from ankle brachial pressure index    measurement

*Assess for patient self care ability in performing daily activities, emotional and psychological stat

*Monitor blood glucose levels, urine output, blood investigations, to identify presence of any secondary infections.

OBSERVATIONS :

*Moist ulcer

*Edema

*Flare in ankle

*Varicose veins

*Hyper pigmentation of affected part

*Shiny skin

*Cool legs and feet

*Gangrenous formation on foot

2. COMMUNICATE :

3. MEDICATE :

* Clean ulcer with normal water or saline solution

* Assess for the location, integrity and severity of infection

* Provide compression stockings

* Keep the wound area moist and clean

* Avoid using topical antibiotics and antiseptics to prevent further inflammation of the affected part

* Motivate for mild exercises for normal function of blood flow

* Administer systemic antibiotic drugs
