6. The action which the nurse should take first is to auscultate bowel sounds . Auscultation of bowel sounds will help to know about any absence of bowel movements and also auscultation is the second most step after inspection in assessment ,without proper assessment and knowing the cause we can give any medications . Palpating the abdomen in pain is done the last because if we palpate the abdomen first it will cause pain and the patient won't allow us to do the further physical examination and tests .
