Case management : it is defined as it is a collaborative and cooperative process in which assess, planning, implementation, coordinating, monitoring and evaluation of the sevices, which are required to the clients to meet the health needs.

Case management is having or main themes or concepts are

* screening

* assessing,

* stratifying risk

* planning,

* implementation,

* follow up

* transitional care

* communicating post transition, and

* evaluating.

These all are important in case management process.

In these screening is the strongest because all persons will come for health screening purpose to maintainance of their health.

Follow up is the weakest because after recovery from the disease so many people will not come for follow up.

Case managers can manage the different types of cases like physical, mental and social and spiritual disabilities patients so that there will be a burn out of case manager due to exhausion that impact of personal emotional, physical and mental behaviour, some times they may feel empty and hope less.

By implementing several steps we can reduces the or prevent the burnout of case manager :

* first identify the problems, the case manager should not burnout he could not identify the problems.

* take the actions according to the problems.

* maintain balance of life and practicing work.

* They need to take care of their health by healthy living life styles.

* getting a peer support is important.

So by maintaining of all these things the case manager can prevent from the burnout affecting.
