5. A blood recipient patient ID system allow for one person verification for blood transfusion at a time to ensure transfusion safety and to avoid blood transfusion errors .

6. Four key facts gathered before transfusion of blood about the blood product in the presence of patient are :-

- name of the patient ( first and last name )

- unique ID

- blood group and type

- date of collection

7. In hemolysis sample , RBC's break and the plasma or serum becomes pink or even red . This destruction of red blood cells is caused either by outside influences or may be an actual indicator of an illness.

Effects of hemolysis :-

high levels of K+ found in RBC as well as LDH; if hemolysis occurs it can cause abnormal results in many tests and also it does not tell about hemolytic antibody .

8. If the patient has an IV line then the phlebotomist must take sample either from the other hand or on the same below the IV line and ask the nurse to stop infusion to the IV line before taking sample .
