Baby Mitchell, 4 months old, is brought to the hospital by his mother. He presents with difficulty gaining weight. His mother states "he just gets so tired when he eats, and sometimes when he is nursing he has bluish color around his lips."

I will concern above with this scenario is,

* difficulty gaining in weight,

* tiered when he eats,

* when he is nursing bluish discoloration around the lips,

* these all the symptoms may seen in the " Tetrology of palate".

* this is one of the congenital heart disease. It includes four conditions those are pulmonary valve stenosis, overriding of aorta, ventricular septal defect and right ventricular hypertrophy,

* due to inadequate oxygen supply this cyanotic problems may arises.

Our next course of action includes :

* educate the mother to go for investigations for the baby like x ray, oxygen saturation measurement with pulse oxymetry,

Boot shaped heart we can find in x ray.

* if it is confirmed educate the mother to take child to surgical procedure.
