Answer: Medical error is one of the major cause that is resorespons for the death in the United States. Medical errors occurs when dispensing medication to the patient occurs improper or inaccurate. Sometimes it generally happens that many of the drug interactions takes place due to medical error, which causes fatal results on the patient. Many people are unable to tackle with the situation.

Medical error could be minimised by using calibration method before dispensing medicine to the patient, proper maintenance of data of the patient such as medical history of the patient, medicines used etc can be documented so that while taking care of the patient, healthcare professionals should know about the present and past condition of the patient. This can avoid any kind of drug reactions or fatal problem.
