In completing the admission history on newly admitted patients, what types of questions should Laura ask related to medications, nutrition, and past history that affect current skin assessment?

Answer: Before any admission of newly admitted patients, many of the questions should be asked to know about the patient's problem and the factors which may cause the problem or due to which problem has arisen. Patient's medical history should be noted as what kind of medication she/ he is taking and how many times per day and also about allergies, including those to medications, topical skin and wound products. Types of food and dietary fibers the patient is taking. The intake of green vegetables and nutrient intake also effects the skin. So, it is necessary that the patient is taking adequate amount of nutrients. The nurse should intervene in the skin changes such as dryness, wounds, rashes, or changes in skin pigmentation etc.checking for any kind of lesions on the skin or anywhere on the body.
