b) Jamal L. is a 9 month old African American infant who is brought to the clinic by his mother because he feels hot and was waking up crying all night. He is the third child and has received regular health care. He woke up in the middle of the night crying furiously. Quieted somewhat when held upright, he was still a little fussy. He took juice from a bottle but refused solid foods. His temp rectally was 38 degrees C, p=152, R=36, wt=9.2kg, ht=29in. He has had no medications. He lives in a two bedroom apartment and shares a room with a 4 year old and a two year old brother. Mr. K. works full time and Mrs. K provides the care along with sitting for her sister's two children. Both parents smoke 1-2 packs daily.

Hyperthermia related to pathological process in the body as evidenced by checking rectal temperature 38 degree celsius.

PES format - P-problem , E- etiology ,S-signs and symptoms

problem-Here child is having high temperature so the problem is hyperthermia

Etiology-It is the cause. It can be due to any pathological process

signs and symptoms-It can be the client complaints or identified clinically.That is the evidence.by checking body temperature.
