12. The ordered dose of potassium phosphate is 18mmol in 250ml of 0.9% NaCl

Available concentration of vial is 4mEq / ml = 3mmol

Means 1 ml of concentration has 3mmol

Then for 18mmol of concentration we should take 6 ml of the concentration

13. The ordered dose of amphotericin B is 0.25mg/kg

Weight of the patients is 154 lbs = 154 × 0.45

= 69.3 kg

So, ordered dose will be 0.25 × 69.3 = 17.3 mg

Available dose of amphotericin B is 50mg in 10ml

50 mg of amphotericin B is present in 10ml

1mg of amphotericin B will be present in 10/50

Then 17.3 mg of amphotericin B will be present in 10/50× 17.3

= 3.46 ml of available drug contains 17.3 mg of amphotericin B.
