patients should be instructed during the home care patient should perform daily light aerobic exercises so that it will help for the reduction of stress.

3.dietary recommendation

-patient should be educated during hospitalization or under the discharge planning about dietary management that pt should have proper combination of fluid intake restrictions, sodium intake and proper dosage of diuretics.

4.telemonitoring

Telemorning should be there to assess the pts diet, weight,sign and symptoms, complications if any developed,drug adherent.
