How many of you have been involved in a FMEA or root cause analysis after an event has occurred? What were your thoughts on both and how many times did you see institutions see them as one and the same?

Thoughts on root cause analysis after an event:

• Humans wants to take easy way out in any difficult situsitua. It is hard to gather meaningful data, dive deep into a problem without prejudice and  apply our reconciled notions as to what is correct.
• Too Often, a team quickly jump to conclusions without having accurate data available. They believe , they have Seen the problem before, so that must be what happned now.
• Assumptions are dangerous and the facts can only be brought to the surface with solid data.
• The Most appropriate thing to do during a root cause analysis with hypothesis is to try every way possible to shoot it down. Look for the defects and errors. If it holds up to intense scrutiny, then you must just have a resolution.

Almost the institutions see both FMEA and root cause analysis essentially the same. The only difference is the technique to identify them.
