What is the professional code and the cpt ICD-cm- codes for this case Presenting Complaint: Head congestion T 38 P 68 R 22 BP 110/72 History: This 33-year-old female established patient comes into the office with congestion, sinus discomfort, and headache. Her ears have been popping. The throat is somewhat sore but she has not been coughing, She is currently on no medication. Physical Examination: She is alert and in no acute distress. HEENT: Ears are clear. TMs pearly bilaterally. Maxillary sinuses are tender with palpation. Nose: Positive rhinorrhea. Pharynx: Positive postnasal discharge. Slight erythema. Neck: Supple and shotty lymphadenopathy Lungs: Clear to auscultation. Abdomen: Soft, nontender with good bowel sounds. Cardio: Regular rate and rhythm Assessment: Acute sinusitis Disposition: Amoxicillin 500 mg one po t.i.d ×7 days. Increase fluids and follow up with me in 7 to 10 days if symptoms are not resolved.
