Presenting Complaint: Motorcycle accident with laceration, left leg T 37 P 72 R 18 History: Patient was involved in a motorcycle accident one hour prior to this ER visit, falling on his left leg and sustaining a laceration to the pretibial area. He experienced no loss of consciousness or head trauma. This was the patient’s first time on a motorcycle, and he was unsure of how to manipulate it. Physical Exam: HEENT: Normal Neck: No adenopathy, supple Lungs: Clear Abdomen: Soft Cardio: Normal sinus rhythm Extremities reveal a well-demarcated pinpoint 5-cm laceration over the mid-pretibial area. The wound is well marginated and discrete with a small amount of grassy debris inside. The depth is penetrating down to the periosteum of the tibia. There is no arterial bleeding, but there is small venous capillary bleeding. Vascular Exam: Bilateral venous pulses in the posterior tibial and dorsalis pedis. Capillary refill time is normal. Neurologic Exam: Normal. Motor and sensory is exam normal, without any paresthesia to the left leg or numbness. Treatment: Irrigated wound copiously with normal saline. Wound infiltrated with 1% Xylocaine. Patient prepped/draped in a sterile fashion. Skin and deeper tissues closed in layers. Skin edges reapproximated with 12 3-0 prolene sutures and alternating vertical mattress interrupted. Skin reapproximated with slight difficulty. Sterile dressing. Bacitracin ointment. Compression dressing. Diagnosis: Acute laceration to the left pretibia, with extension down to the periosteum Disposition and Condition of the Patient at Discharge: Patient instructed in wound care precautions. To return tomorrow between 1:00 p.m. and 11:00 p.m. for recheck. Patient given prescriptions for Tylenol #3, 1 to 2 tablets prn every 4 to 6 hours for pain; Keflex 250 mg, 1 per mouth, 4 times daily.
