PREOPERATIVE DIAGNOSIS: Arteriosclerotic heart disease.

POSTOPERATIVE DIAGNOSIS: Arteriosclerotic heart disease.

PROCEDURE: Coronary artery bypass graft x 5.

The patient was brought into the operating suite and placed in the supine position. Anesthesia was administered, and monitoring lines were placed. The patient was prepped and draped in the usual sterile fashion. The sternum was opened through a midline incision. The saphenous vein was harvested from the left ankle. After the sternum was opened, the pericardium was incised and the coronary field was carefully examined. The mammary artery was taken down with cautery, and a drain was placed in the pleural space. Cardiopulmonary bypass was started. The patient's core temperature was dropped to 30 degrees. The aorta was cross-clamped, and cardioplegia was administered. The acute marginal branch was grafted with continuous 6-0 Prolene and the saphenous vein. The graft was brought up to the ascending aorta. The heart was turned over; significant disease was seen in the proximal PDA. A segment of saphenous vein was sewn in with 7-0 Prolene. The mid OM was opened; an anastomosis was created. The graft was brought up to the aorta. Then the diagonal branch of the artery was grafted side-to-side with the left mammary. The distal LAD was opened and grafted to the tip of the left mammary. The patient was rewarmed, and the heart pinked up. The vein graft clamps were released and filled well. There was no distal or proximal bleeding. The patient was paced into normal sinus rhythm. The patient was disconnected from bypass. Two chest tubes and a drain were placed. The aortic site was closed with oversewn sutures. The area was irrigated with antibiotic solution. The entrance incisions were closed with Vicryl, and the leg, was wrapped. The sternum was closed with wire and layered suture closures. The patient tolerated the procedure well and was transferred to CICU.
