CPT procedures with Modifiers.

    1) 64721 - Neuroplasty or transposition : Median nerve at carpal tunnel.

         Modifier: 50 - Bilateral procedure.(Right and left wrist).

    2) 43235 - Esophagogastroduodenoscopy,flexible,transoral : Diagnostic.

         Modifier: 53 - Discontinued procedure.

3) 28485 - Open treatment of metatarsal fracture,includes internal fixation

                      when performed each.

         Modifier: F9 - Right hand,fifth digit.

    4) 93926 - Duplex scan of lower extremity arteries :Unilateral study.

         Modifier : RT - Right side (Right leg).
