As per the scenario, the patient is conscious and oriented. If the nurse find an abnormal ECG rhythm at this time, she should check the device for malfunction, and adjust the leads to view the rhythm and also, she can ask a coworker to confirm it. Announcing code blue is not appropriate as the patient is responsive and alert. Continue to observe the same rhythm is not appropriate as it is important to rule out the cause of abnormal rhythm as soon as possible to prevent undesired complications. as patient is awake and responsive, the last option also inconvenient.

Thus the answer is, Readjust the chest leads to view the pacer better and ask co worker to confirm the lead placement.
