* Name of the patient.

* Age and weight of the patient.

* Drug name

* Dosage form of the drug ( e.g., tablet, injection or inhalants.)

* Exact strength ,dose or concentration.

* Dose, frequency and route of the drug.

2. Tha 4 parts of prescription are:-

a) Superscription :- The superscription is represented by a symbol, Rx, which is always written at the beginning of the prescription.

b) Inscription :-This is the main part of the prescription. It contains the names and quantities of the prescribed ingredients.

c) Subscription :-This part of the prescription contains prescriber's directions to the pharmacist regarding the dosage form to be prepared and number of doses to be dispensed.

d) Signature :-It is usually abbreviated as "Sig" on the prescriptions and consists of the directions to be given to the patient regarding the administration of the drug.

3. Following is the list of information that must be includes inc all prescriptions:-

* Name and adress of the prescriber and phone number( if available).

* Information of patient ( e.g., name or age)

* Date of the prescription

* Name and strength of the drug.

* Route by which medicine to be taken.

* Frequency of medicine to be taken.

* Signature or initials of the prescriber.
