What is the first question or questions you should ask this patient prior to starting your assessment in order to build trust with this patient? 2. Does this patient require parental permission to obtain contraception? If her parents ask, are you allowed to tell them about her contraceptive choices or visit? 3. As a part of your assessment, what part of the physical exam do you need to perform this initial visit? Is a pap smear necessary for this patient in order to prescribe contraception? 4. List three forms of contraceptives that would be appropriate for this patient and give several risks and benefits for each, as if you were educating the patient. 5. List the contraceptive that you would most want this patient to use and give me three reasons why.

2.Yes we need the consent from the patient and family

3.No. its asper the physician order

4.Taking the porper physical history and examination   including vitals and weight& height and breast & cervical examination

5 For the initial assessment first we  should do the pregancy test

6 In history should take the history of cardivascular diseases incculding the blood pressure& lipid profle.then the history of liver diseases renal historyand any bleeding disorder

Yes it is neceesary for the pap smear test because it can help the early detection & prevention of cervial cancer

barrier method like condom|/ diapharmor cervical cap and orall contraceptives likes hormonal pills reason is that for the safest
