How would you educate the patient and his wife about the situation today? the patient is taking St. John’s Wort and Coumadin
Despite taking warfarin ,INR (International normalized Ratio) is less than the reference because St John's wort causes induction of CYP enzymes .Thus the metabolism of the coumadin is increased hence its effectiveness is decreased.Thus he has to stop taking St John's wort.
