One of the anxiety disorder is post traumatic stress disorder. Exposure to traumatic event is required for a diagnosis of PTSD. The person have witnessed , experienced, or have been confronted with a situation that involved definite or threatened death. The patients response to the trauma include intense fear, helplessness or horror.
The two alterations are neuroendocrine theories and neurochemical theories.
Neuroendocrine theories:
Neuroendocrine changes in the hypothalamic pitutary adrenal axis are implicated in the phathophysiology of PTSD. Normally the immediate response to stress occurs as an automatic response from the amygdala to the sympathetic and parasympathetic systems and the HPA axis. The release of vortico tropin releasing factor stimualates cortisol secretion from the adrenal gland. Both catechol amines and cortisol levels rise in tandem. Cortisol reduces the stress response by tempering the sympathetic reaction through negative feedback on the pitutary and hypothalamus. Dysregulation of HPA acis is postulated to be risk factor for eventual development of PTSD.
Neurochemical theories:
5-HT and NE are associated with the processing of emotional and somatic contents of memories in amygdala. The alarm centre, the locus ceruleus, release NE to stimulate the sympathetic and parasympathetic nervous system.dysregulation of the processing of sensory input and memories may contribute to the dissociative and hypervigilant symptoms. Preliminary data indicate that the 5-Ht and 5- HT2 antagonist meta chlorophenylperazine causes increased anxiety symptoms in patients with PTSD.
