. For each of the following patients, the order on the Electronic Health Record reads “Oxygen therapy – keep oxygen saturation greater than 94%”. Name the oxygen delivery device that is needed for each patient and provide rationale.(6 points)

  1. 61-year-old male is S/P CABG x3. His hemoglobin is 6.9. He has been ordered 1 unit packed red blood cells. His oxygen saturation upon assessment is 92% on room air and his lungs are clear to auscultation.

  1. 58-year-old female with congestive heart failure that was admitted with severe fluid overload. After aggressive treatment with diuretics, she states she is feeling much better and her oxygen saturation is 100% on a nonrebreather mask. She has fine crackles just at the base of her lungs bilaterally.

  1. 24-year-old male construction worker who fell off a roof on the job. He broke his right femur, 2 right ribs (with resulting pneumothorax and chest tube insertion), and his nose which is now packed with gauze. His oxygen saturation upon assessment is 90% on room air.

Answer #1

A) A Venturi Face Mask will be used in this case as it would provide the accurate oxygen needed by the patient with its different colour codes .

Venturi mask :-

Concentration of oxygen varies from 24% to 40% or 50% at flow rates of 4 to 10 L/min. Has wide bore tubing and color coded jet adapters that correspond to a precise oxygen concentration and liter flow

B) No oxygen delivery device is needed because the saturation is 100% , a saturation above 92% requires no oxygen administration .

C) A Nonrebreather Face Mask will be used in this case , because the patient requires highest oxygen concentration due to the complications associated with fall.

Nonrebreather mask :-

Concentration of oxygen is 95% to 100% at flow rates of 10 to 15 L/min. It delivers the highest oxygen concentration possible. One way valves on the mask and between the reservoir bag and the mask prevent the room air and the client's exhaled air from entering the bag so only the oxygen in the bag is inspired.
