Interventions:
I). Use pen and paper method to communicate.
II). Attach call bell bell to patient bed side it helps to call the nurse if any need.
III). Use other modes of communication like message,sms and mails.
IV). Talk to the patient slowly.
V). Always stand straight to the patient and talk.It helps to read your lips.
Positive outcomes:
This ways promote effective communication.
Negative outcomes:
If the patient is semiconscious or illiterate all this modes are useless.
Evaluation:
Patient can establish communication with care provider and others.
Nursing diagnosis 2:
Ineffective cerebral tissue perfusion related to impairment in the brain region as evidenced by aphasia.
Interventions:
Place a board and marker at patient bed side.
Give collection of pictures of daily needs and activities to patient bedside.
Give speech therapy to the patient.
Give physiotherapy.
Take medications as prescribed by the physician.
Positive outcomes:
Patient gets improvement in the cerebral tissue perfusion.
Negative outcomes:
If patient is semiconscious all are useless.
Evaluation:
Patient can maintain effective communication .
