Nursing Diagnosis;
AEB: Fever of unknown origin (FUO)
Failed to find out the etiology during 3 other instances.
R/T undiagnosed
AEB; Signs of dehydration such as tented sternum, sunken eyes, weight loss.
R/T dehydration due to hyperthermua
Interventions:
Tylenol medication
every 4 hours monitor the temperature
IV fluid for dehydration every 2 hours
Bed rest
Cold compress and stay cool
Evergy level monitoring
Positive outcomes:
Fever reduces with body temperature coming down to 38.5 degree
Negative outcome:
Etiology could be infection, malignancy, nosocomial or immune deficiency syndrome., drug induced
Evaluation;
To study the fever pattern
CBC blood picture especially neutrophil count
Comprehensive medical history.
HIV serology testing
Chest x ray
LFT
Electrolytes
Blood & urine culture
urine analysis
Skin PPD
CT scan of abdomen and pelvis
Finally assign to most likely category for further evaluation such as malignancy, autoimmune, infection and miscellaneous etc
