Pacemaker maintains heart rate during bradycardia, it has a sensor who senses your heart rate, if the rate slows down then the pacemaker starts giving impulses which again raise your heart rate to normal and it stops giving impulses when your heart has impulses to beat on his own. so when a patient is having Ventricular tachycardia the pacemaker has nothing to do.
ICD's (Implantable cardiac defibrillators), not only sense when you have bradycardia it also senses the tachycardia it deliver impulses during bradycardia and delivers shock during tachycardia.
