In her social history, her husband is deceased from AIDS one year ago with progressive multifocal leukoencephalopathy and Kaposi sarcoma. She denies any history of intravenous drug use or transfusion and identifies three-lifetime sexual partners.

Her past medical history has several episodes of diarrhea, sinusitis, thrush, and vaginal candidiasis. She gave a history of a 10-pound weight loss. The chest x-ray showed diffuse lower lobe infiltration. She was admitted for bronchoscopy with alveolar lavage to confirm the diagnosis.

1.i think the patient have

Two diagnosis:

  1. tuberculosis - respiratory disease mainly affect the lungs first and extends to other area.
  2. Pneumonitis - lungs tissue gets inflamed.

2.two diagnosis of husband:

  1. AIDS-Acquired immuno deficiency syndrome.
  2. Kaposi sarcoma.

3.four disorder:

  1. Chest pain,
  2. Difficulty in breathing,
  3. Fever and
  4. Cough.

4.x-rqy finding:

Lower lobe infiltration- pus or fluid or blood get accumulated in the lower lobe of the lung.

5.next step to the patient;

alveolar lavage for the confirmation of diagnosis and give medical treatment for the disease and rehydration for fluid loss and pain killer to reduce chest pain.
