PATIENT EDUCATION WHO RECIEVING ANTICOAGULANT THERAPY ;

MEDICATIONE  ACTION;

*Warfarin is a medicine that cotrols how your blood clots.It is used to help prevent blood clots that may cause serious health problems.These problems include heart attack{myocardial infarction},stroke,a blockage in an artery or vein {thrombus}, or a blood clots that travels to the lungs {pulmonary embolis} ,atrial fibrillation.

Before you start taking this medicine,tell your health care provider if you have any of these conditions:

-Stomach ulcer now or in the past

-Vomited blood or had bloody stools

-Aneurysm,pericarditis ,pericardial effusion

-blood disorder

-Recent surgery , stroke , mini- stroke,or spinal puncture

-Kidney or liver disease,uncontrolled high blood pressure, diabetes,vasculitis , heart failure, lupus or other collagen vascular disease , or high cholestrol

-You are pregnant or breast feeding

-You are younger than 18 years old

-Recent or planned dental procedure

Although warfarin reduces the risk of blod clots ,it increases the risk of your bleeding. Because of this you must take the medicine exactly as you are told by your health care provider.

*DOSAGE OF WARFARIN;

-Take your medicine at the same time everyday

-take your medication with or without food [if it causes stomach upset , take it with food]

-take your medications exactly as prescribed

-The doctor or pharmacist who is treating your conditions will determine the correct and suitable doseof warfarin .Each patient his or her own dosing regimen.

*ADVERSE REACTION OF WARFARIN;

When you call your health provider

-sever bleeding, including hevier than normal menstrual bleeding

-Red or brown urine

- black or bloody stool

-severe headache or stomach pain

-vomiting of blood materials that looks like coffee grounds

-coughing up blood

-bruising that develops without an injury you remember

-dizziness or weakness

-vision changes

-head injury, even if you are not bleeding

-joint pain, discomfort or swelling ,especially after an injury

COMMON DRUG THAT CAN INTERACT WITH WARFARIN INCLUDE;

-Aspirin or aspirin containing products

-Acetaminophen or acetaminophen containing products

-antacids or laxatives

-many antibiotics

-antifungal medications such as flucanazole

-cold or allergy medicines

-ibuprofen

-medications that treat abnormal heart rhythm such as amiodarone

MONITOR OF INR

PT-INR ttesting.the specific blood test used to measure the time it takes for blood to clot is called aAVOID prothrombin time test.The INRis a calcution based on results of a PT test .and is used to monitor individuals who are being treated with the warfarin

DIETARY AND SAFE CONSIDERATIONS;

-take/ food vitamin k containin.

-donot go bare foot alwats bear shoes

-avoid alcohol
