Ans) Discharge planning is the process of identifying and preparing for a patient's anticipated health care needs after they leave the hospital. Hospital staff cannot plan discharge in isolation from the patient and family.
- Discharge planning is an interdisciplinary approach to continuity of care; it is a process that includes identification, assessment, goal setting, planning, implementation, coordination, and evaluation and is the quality link between hospitals, community-based services, nongovernment organizations, and carers.
- The discharge plan helps the patient develop a workable, realistic care plan following discharge that is fiscally appropriate for patient, provider, and payer. The initial patient interview allows for the early identification and management of patient needs.
