Core Measures is a process which deals with the quality of care provided by the hospitals which works and follows the guidelines of Centre for Medicare and Medicaid Services.The measures dealt are both inpatient and outpatient.Eg Inpatient measures such as Sepsis, Stroke, VTE, Global ED, PC Mom, PC Baby etc.
Sepsis : Identifying Severe Sepsis and Septic Shock ,and the treatment provided with in 6hr and 3hr bundle time frame is one part of the work to be done by the employee working for the measure.This is a concurrent work.If any bundle is missed by the hospital to perform, alert would be sent to the hospital about the missing bundle.So that hospital can improve the quality.
Here company had decided to intimate the hospital with in 48 hrs. of the fallout parameters.Examples of missing parameters or bundles are lactic acid, Blood cultures, antibiotics, fluids, vassopressor administration and tissue perfusion.If any of these bundle parameters were missed that particular case is considered as Fallout.Here as per the decision of the company all the alerts were sent with in 48 hrs.
Discussion : The part of the analysis failed here is Strategy.
Rationale: The decision of working concurrent for these type of situations does not show any effect.This can be possible only with live process.In concurrent we may work on the cases which got admitted 1 week back diagnosed as Sepsis 1 day back meeting Severe Sepsis criteria and by the time we work on the case it had missed the total bundle time frames and it is of waste of sending alert where hospital can no more show their quality of treatment beyond the time frames and neither patient nor hospital are benefited.
Differently the process has to be run in live , so that patients who met Severe Sepsis criteria with missing bundles can be alerted with in the time frames .So that hospitals can take immediate action.
SWOT Analysis : Strategy, Weakness, Opportunities and Threats can be better identified in this process.
If company had logical thinking on the strategy they applied , the weakness of sending alerts after the time frames can be identified.So that theý can search for other opportunities as I suggested the process of live work.So that the threatens can be avoided in this scenario.
