1a.) Beneficial effects related to HRT that should be discussed with women during perimenopause:
HRT can also improve sleep, muscle aches and pains and quality of life in symptomatic women.
HRT can improve your mood and also depression. It is also helpful in cognitive behavioural therapy.
Potential risks related to HRT that should be discussed with women during perimenopause:
Small increased risk in women taking oral oestrogen-only or combined HRT. Tibolone increases the risks of stroke in women.
Oral HRT increases the risk of VTE. These risks increases with age and with other risk factors, such as obesity, previous thromboembolic disease, smoking and immobility.
Nursing Education for HRT:
Patient should be encouraged to assist in their dosage regulation. They should report symptoms of too much esrogen, including irritability, mood disturbances, headache, nausea, bloating, fluid retention. Too much progestin includes irritability, and gastrointestinal distress.Any abnormal vaginal bleeding, blood pressure elevation, visual change, severe headache, abdominal pain, chest pain, dizziness needs immediate medical attention.Nurses should counsel the patient to reduce weight, a low fat diet, high fibre diet, an intake of 1500mg calcium per day, monthly breast examination, pelvic examination.
1b.) Transurethral microwave therapy is a method to treat urinary problems due to enlarged prostate.
A small microwave antenna is inserted through the tip of the mens lowerpart in to the tube that carries urine from your bladder.
The antenna emits a dose of microwave energy that heats and destroys prostate and improve urine flow.
TUNA is also known as radiofrequency ablation. it is a procedure to treat urinary problems caused by enlarged prostate.
TUNA is known to be safe therapy for BPH. TUNA is effective and good long term clinical improvement. TUNA treatment stands the test of 5 yr follow up with low failure rates. According to the study more than 75% of the patients do not need additional treatment for BPH.
Nursing care for the patient with TUNA and TUMT:
They need to monitor urinary output to prevent complication, the nurse should teach the patient exercises to regain urinary control. Patient should avoid activity like straining and heavy lifting, patient should be taught to avoid spicy foods alcohol and coffee, Nurse should instruct patient to drink enough fluids.
