Diagnosis code is correct as the problem clearly mentions unilateral (left) inguinal hernia.

Unilateral inguinal hernia which is recurrent but without any obstruction or gangrene the ICD 10 code is K40.91

Unilateral inguinal hernia which is non recurrent and without any obstruction or gangrene the code is K40.90

Bilateral inguinal hernia which is non recurrent and without any obstruction or gangrene the code is K40.90

Procedure code reported is wrong. The workbook clearly mentions procedure with per-cutaneous endoscopic approach for which correct code is 0YQ64ZZ (and not 0YQ60ZZ which is for open approach).
