Answer: Mrs. Smith stays at home on the blue ridge mountains. She has cardiovascular disease which caused her to get into hospital for four times. She also have hypertension and diabetes. She is unable to see properly as her eyesight is getting worsen with the time. So these are the things which causes problems to her staying at home safely.

It is the need of the patient as she can't go to the hospital all the time. So telehealth application will help her to overcome with some of the problems that she is facing such as she forgets to take medication on time or consulting to the healthcare professionals will be easy for her.
