The two more potential stakeholders could be

1. Nursing personnels

2. Health insurance companies.

Each stake holders and their stakes and reactions:.

1. Patients - interested to have change( stake)- the patients are interested to have change in practice if it is to improve the quality of health care and efficiency of team members. Customers always expect a change which would be beneficial for them. And obviously they will be happy to receive a cost effective high quality care.

2. Referring physicians- not interested (stake) - they won't be much interested to have change in practice because they are adapted to the freezer old practice and employees always have a resistance towards change. They will be confused about the success rate of change and would be concerned with the criteria of changes which may indirectly affect their comfort zone.

3. Northside community hospital- interested ( stake ) - community hospitals are always interested to welcome the new practices as they like to expand.changes are inevitable and change of practice will be appreciated to develop the community hospitals.

4. Other plastic surgeon in the town - anxious ( stake ) - they may be anxious about the change of practice. If this changes become successful, obviously it would affect the practice of other surgeons in the town.

5. Nursing personnels- not interested (stake) - they will have lot of anxiety and worry towards the new changes as they are also adapted with the old practices. When the practice is getting changes their work responsibilities also changes and they would be the one to convince and educate other stakeholders specially patients about the change. So they may wish to continue the same practice.

6. Health insurance companies- not interested (stake) - the reaction of health insurance companies are actually unpredictable but most of them may resist the change because they have to review their policies and procedures. The standard operating policies has to be revised according to the change in practice which would be hurdle for them .

The most influential stakeholder would be patients . The policy makers have the commitment to provide beneficial services to the customers that is patients . Patients are the valued customers of a health care organizations and every government is standing for the benefit of them. So I feel Dr.cooke has to analyze clearly about the stake and reactions of patients before finalizing with the plan. If the change in practice is not beneficial for them , it won't be having a successful end once implemented.
