Answer: Nausea can occur due to many of the reasons. The care plan for the nausea includes:

1. Assessment: Assessment is performed to know the reason or the cause of the nausea. Collecting information i.e. subjective and objective data.

2. Measurement of symptoms: In this measuring the intensity of the symptoms such as feeling of nausea could have high or low intensity. Any other symptoms that may be the outcome due to nausea.

3. Antecedents and intervention techniques: Looking for the cause such as medical history, food allergy, food poisoning etc. Because these can be the reason of the nausea.

4. Measures for intervention: If the nausea is due to the the side effects of medicine then it's symptoms will go on its own. But if it is due to excessive amount of food or any kind of food infection, medication should be taken.
