The patient is experiencing symptoms of breathing difficulty at rest, orthopnea, paroxysmal nocturnal dyspnea and pedal oedema.

The above seen symptoms take place in following five diseases:

1. Pulmonary edema :

2. Congestive heart failure

3. Chronic obstructive lung disease

4. Valvular heart disease

5. Pneumothorax

These conditions are characterised by increased ventlatory demand, decreased ventilatory capacity, increased airway resistance, reduced pulmonary compliance, reduced cardiac capacity and reduced tissue perfusion, pulmonary congestion. All these induces the above symptoms.
