Mrs. Watson, an 81-year old, is scheduled to be admitted to your rehabilitation center from the hospital for short-term rehabilitation. She is part of a care continuum program for patients who have undergone hip replacement surgery. The program philosophy recognize that each patient experiences the illness differently and strives to develop and implement an individualized plan of care that will enhance recovery and overall sense of wellness.

On admission Mrs Watson tells the nurse,"I am nervous about being here. So much is at stake-if I don't do well they won't let me go home by myself.I was so used to taking care of myself before I fell and broke my hip. This has been hard.Except for some joint stiffness in the morning I really don't have anything wrong with me and I am proud of that ! I have never had to take any regular medications. I have always been a can-do person and going all the time; active in my church, volunteer at the senior center to help with elderly folks and help watch my great-grand baby who just turned 1-he is a handful ! Now I feel like I can't take care of myself anymore and I am afraid this will happen again. It has been hard to deal with the pain and discomfort of both the injury and the process of getting well. I have never thought of myself as 'old and feeble until now and I don't like it. Only old people use walkers ! I have got so much I still want to do !"

1.The factors which contributed to disruption of her health are

• A fracture prior to surgery
• Hip replacement surgery which has detoriate her health conditon comorbid to age
• Psychological or emotional disturbance of not being able to do her routine daily works

2..There are five stages of illness (symptoms, sick individual, healthcare, patient recovery or rehabilitation.

This patient is in the stage of the recovery or rehabilitation .As she as undergone surgery.This is the period of getting back to her normal life of being able to walk

3.The factors which were being affected by her illness are

• Inability to be active in church
• Unable to help the elderly population in senior center
• Unable to play with her great grand children
• In general her daily activities has been greatly affects due to her illness
