What initiatives have accreditation organizations and other external agencies undertaken to address these issues? What strategies have been used by ancillary healthcare institutions (mental health and public health) to reduce medical errors? Do you think these efforts have been effective?

Medical error is an important issue that increases mortality. The step all frequent errors in Medicare and Medicaid Act of 2000 establish patient safety within the Agency for health care research and Quality establish medical error detection and prevention system for the hospital with Medicare and Medicaid programs, it includes error-detection system, strategies to correct errors, root-cause analysis. This information should be reported to state public health departments, accrediting irganization or medicare peer-review organization. poor safety performance for more than 2 years that is noncompliant facility should be reported to federal officials, to release medical error records to the public.
adverse health outcomes due to a medical error can be prevented by patient safety precaution and strategies. Patient safety is the main goal for many health care facilities, ancillary nurses in mental health facilities focus on first do no harm. the traditional goal of health focus on multifaced care reduces the medical error. safety culture risk management with a successful system, knowledge, respect, confidentiality, and trust can may change and protect patient safety and improve quality of care. it also minimizes the risk and increases patient safety among mental ill patient and public health...these all the effective measures which can improve patient safety.
