Fatima is a 35 years old woman, has always been a moderately dynamic person, cheerful and happy, she did have mood swings and temper tantrums. sometimes she become angry, and insults other persons. she enjoyed many activities such as dancing, singing. She loved to dress in expensive clothes with extravagant colors. She was well-liked but had few close friends. there were mental disorders on Fatima's father side of the family. She was married when she was 23 years of age to a man who is 17 years older than her. the marriage lasted three years and after a big blowup, the husband left and moved to Kuwait. He stated that Fatima was selfish and demanding, always wanting her own way. She moved to a new neighborhood and a small apartment. She thought a change might help her adjust to a new life without her husband, but after 3 months she began to complain of anxiety and "nervousness."Her family noticed that she was becoming more hyperactive. They approached her about seeking help, but she responded by throwing glasses and dishes around the apartment. They called the police and with her approval, she was admitted to the psychiatric hospital.

Assessment is the collection of gathering information.
Subjective data includes information from the patient, family or caregivers by interviewing them. The patient has a history of mental disorder from the father side. she is a dynamic personality. she is more hyperactive.
Objective data includes observation and measurable by vital signs, physical examination. the patient was anxiety and nervousness.
Nursing diagnosis with goals:
   The risk of suicide related to anxiety and nervousness
Ineffective coping related to hyperactive
Goal:
prevent risk activity by reducing the symptoms
Encourage patient positive self-talk and coping strategy
Nursing intervention:
The risk for suicide:
- Talk with the patient to know about self-injury
- Observe for any risk factors
- collect patient history for suicide attempt
-check the patient for any substance use
- check patient history for a mood disorder
-promote safety precautions and support resources
- assess patient decision-making power.
-Stay with the patient and help them with problem-solving.
-educate the patient about medication to cope with the ability
- Encourage family and friends to stay with the patient.
Ineffective coping:
- promote a working relationship with the patient to reduce the isolation
- Identify patient personal skills and knowledge.
-allow the patient to express their feelings.
-avoid false reassurance
- Encourage patient for physical ability like watching television, reading books and exercise
- Provide touch therapy with the same gender to relax and make comfortable.
-Avoid external stimuli to decrease anxiety
-Encourage patient for cognitive behavioral relaxation therapy like music
-allow patients for counseling to help develop problem-solving skills.
