In the present case study, we are discussing Lillian Tudor, who is 84 years old and is taking Tramadol for stiff knees and sore joints.
1. Captopril is used to treat high blood pressure (hypertension), kidney problems caused by diabetes. While administering captopril blood pressure will need to be checked often, and may need frequent blood tests. Encourage him to drink plenty of water each day while taking captopril. Sometimes this medication can cause false results with certain medical tests.
2. Captopril is an ACE (Angiotensin Converting Enzyme) inhibitor.
Adjust doses with renal impairment
Administer one hour prior to feeding (food will decrease absorption)
May cause false positive acetone in urine
Action potentiated when given with other hypotensive agents and diuretics
Peak: 30 - 90 min.; duration of action is usually 2 - 6 hours, but may be significantly longer (>24 hours).
3. Lillian is taking Captopril for treating his high Blood Pressure.
Chief Indications
a. Treatment moderate to severe hypertension.
b. Afterload reduction in patients with CHF
4. Loop diuretics may increase the effects of the ACE inhibitor and the angiotensin II receptor antagonists.
How the interaction occurs and the cause of the interaction is not known
What might happen:
The blood pressure may decrease, causing weakness, dizziness, or fatigue. This is more likely to occur during the first few days after the ACE inhibitor or angiotensin II receptor antagonist is added to your diuretic.
