Digoxin is a drug which gives positive ionotropic effect it acts on the ATP bind by infusing calcium in to the myocardium which helps in the contractility of the heart .

A digoxin level of 0.3-2.6 is considered as therapeutic value anything above leads to digoxin toxicity.

During hypokalemia the inhibitory effect of digoxin is increased because it acts at the ATP bind exactly at the site of potassium so when postassium is low digoxin increases its inhibitory effect which can lead to arrythemias and sometimes death

Hydrochlorothiazide is a potassium sparing diuretic which has a serious interactions with digoxin increases the risk of hospitalizations digoxin toxicity

Potassium chloride should be administered IV diluted in a solvent infused over a particular time .this explains that it cannot be administered IM or SQ because a large quantity of medication cannot be administered SC or IM .giving Potassium IV push leads to the client recieving too much potassium suddenly which leads to increase contractility of heart arrythemia and death

The nurse instruct the client to always dilute oral potassium in water before consuming .she should instruct the client to dilute 10ml of oral motassium in 100ml if water before consuming.
