Incident: Unplanned interruption to a service, a reduction in the quality of a service or an event that has not yet impacted the service.

Injury reporting: In a health care facility, such as a hospital, nursing home, or assisted living, an incident report or accident report is a form that is filled out in order to record details of an unusual event that occurs at the facility, such as an injury to a patient.

b.Hazard identification is part of the process used to evaluate if any particular situation, item, thing, etc. may have the potential to cause harm.

Risk assessment is a term used to describe the overall process or method where you: Identify hazards and risk factors that have the potential to cause harm (hazard identification).
