* Calculation of ml/hr.

     Formula

         Flow rate (ml/hr) = Volume(ml) /Time(hr)

Calculation of volume

        Formula = Desired dose/Dose in hand * Quantity

    Here

          Ordered dose = 2mEq/kg/4 hours.

           Patient weight = 150 lbs.

       Convert lbs to kg for calculation

                    1 lbs = 0.45 kg.

       So

         Patient weight = 68 kg.

   Desired dose (D) = 2 mEq * 68

                                                        = 136 mEq.

                          Dose in hand(H) = 150 mEq.

                                 Quantity(Q) = 1000 ml.

     As per formula

        Volume(ml) of drug required = D/H* Q

                                                          = 136/150 * 1000

                                                          = 0.9067 * 1000

                                                          = 906.7

                                                          = 907 ml.

                         Time(hr) = 4 hrs.

            Flow rate = Volume(ml) /Time(hr)

                            = 907/4

                            = 226.8

                            = 227 ml/hr.
