In source oriented medical records data is arranged depend on the source .It is a conventionel method of organizing data.The suppliers can be doctors or nurses who is involved to give according to the specialities in each department.

Problem oriented medical records are considered as successfull method.because,it can give a structure that enables physicians to record their note according to the patients codition.this provides a better information about the history of the patient.it is easy to acess in all situations.

In source oriented medical records each health care providers such as doctors,nurses,and dietician can chart their notes in the file which are seperated for each section.this charts are separeted by tabs.

Charts are divided i to four section in problem oriented medical re ords.it is depend on data base,problems,careplans and outcome.All health workers can write one problem depends on tbeir speciality and give result regarding the issue.

I think problem oriented medical records can be convienient for each health worker.only we need to ente the issue and it progress note in the sections.it is easy to refer in emergecies.moreover,time saving is possible while doing this method.when it reads everyone get a clear picture of patients present and past condition.
