ANSWER.

* Calculation of dose required for the patient .

       Ordered dose = 100 mg/m2.

   As per nomogram

       Height of the patient = 177.8 cm (70 inch).

       Weight of the patient = 72.6 kg (160 pounds).

BSA (body surface area) of the patient (m2).

     Formula = (Height(cm) * Weight(kg)) / 3600.

As per formula

          Body surface area of the patient = 177.8 * 72.6 / 3600.

                                                             = 12908.3 / 3600

                                                             = 3.59 m2.

     So

        Dose required for this patient = 100 * 3.59

                                                    = 359 mg.
